Recording & Reporting

When we have concerns, it is important that we record them and pass them to our DSL. You do NOT need consent to discuss a concern with your DSL or your Line Manager.

When making a record of safeguarding concerns we should ensure that:

  • The record is clear, and that the person reading it will understand what is meant, even if they have no prior knowledge of the child or the situation
  • The record is legible and understandable when it is hand written
  • The date and time are included, and that the document has been signed and dated (electronically signed is fine.)
  • Who what where when? Who was involved, what happened and when did the events take place.
  • Who was present or witnessed this?
  • The record is objective and factually accurate
  • It clearly identifies implications, interpretations and conclusions drawn from the facts as such
  • It clearly outlines any discussions, advice or actions that have already taken place
  • That it outlines what advice was given, by whom, and when?
  • It outlines the thought processes and rationale behind decisions any decisions
  • Where possible, quotations, body maps (but not photographs) etc are used to support of communications and understanding
